To book in for our services, you will need to first complete the Online Patient Questionnaire Form per the link below (please attach your medical referral through that link).

Alternatively, you can download (per the ‘Hard Copy’ link below), print, complete, scan and email the form (along with your medical referral) to admin@surfocastendoscopy.com

Once received, a SurfCoast Endoscopy team member will be in touch to finalize the next steps.

Day of Procedure:

Please bring the following:

 

  • GP referral
  • List of medications
  • Health insurance details, Veterans’ Affairs card, Medicare card, and/or Pension card
  • Expect a four-hour stay from admission to discharge.

Fee Information:
  • Private insurance holders: Pay any excess or co-payments before admission.
  • Non-private insurance holders: Pay hospital, proceduralist, and anesthetist fees at admission. The Health Precinct will provide cost details.
  • Medicare reimburses parts of the proceduralist and anesthetist fees.

After the Procedure:
  • Patients are evaluated by their doctor, receive discharge instructions and should arrange for a follow up consultation at a future date with their primary medical practitioner (i.e. referring GP) to go through results.
  • Do not drive or operate machinery for the day. Arrange transportation and overnight company.
  • Normal activities can resume the next day.
