The Endoscopy Exclusion Protocol
To ensure the safety and well-being of patients undergoing day hospital procedures, it is imperative to adhere to the following protocol when referring patients. Our goal is to provide efficient, high-quality care while minimizing risks associated with day procedures.
Pre-Anaesthetic Consultation (PAC) Requirement:
A Pre-Anaesthetic Consultation is mandatory for patients presenting with any of the following conditions:
- Body Mass Index (BMI): Patients with a BMI greater than 39.
- Age Considerations: Individuals aged 75 years and above.
- Mobility Concerns: Patients identified as fall risks.
- Allergies: Patients with multiple allergies.
- Anaesthetic History: Those with previous complications related to anaesthesia.
- Cardiac Conditions:
- History of heart attacks.
- Presence of stents.
- Use of blood thinners.
- Atrial Fibrillation (AF).
- Pacemaker implantation.
- Prior cardiologist reviews.
- Respiratory Issues:
- Chronic Obstructive Pulmonary Disease (COAD).
- Asthma.
- Lung Cancer.
- History of lung surgery.
- Cystic Fibrosis.
- Other significant lung diseases.
- Renal Impairment: Patients with kidney failure or significant renal impairment.
- Diabetes: Both Type 1 and Type 2 Diabetes, requiring medication review.
- Blood Thinners: Patients on anticoagulation therapy requiring dosage adjustment or review.
- Recent Hospitalizations: Patients recently hospitalized for any medical condition.
- Infectious Diseases: Patients with Covid-19 or other contagious diseases.
- Mental Health: Patients with acute psychotic episodes, psychosis, or behavioural issues that may impact care.
Exclusion Criteria:
The day hospital is unable to accommodate patients with the following conditions. These patients should be referred to a public hospital for their safety and comprehensive care:
- Body Mass Index (BMI): Patients with a BMI over 45.
- Weight Considerations: Individuals weighing more than 140 Kgs.
- Paediatric Patients: Patients 17 years and under
- Mobility: Non-ambulant patients.
- Self-Care: Patients unable to care for themselves, including but not limited to:
- Cognitive impairments that may lead to harm or an inability to follow pre and post-procedure instructions (e.g., neurological, or developmental disorders).
- Patients lacking understanding of their healthcare rights.
- Patients requiring assistance with mobility or Activities of Daily Living (ADLs).
- Patients at risk of aggression or severe delirium.
- Infection Control: Patients identified as contacts for Carbapenem-Resistant Enterobacteriaceae (CRE) or requiring airborne transmission-based precautions.
